waitingroom1Preparing for your office visit is a very important part of ensuring that we are able to meet all your needs and provide you with the highest quality of care.

Patient Portal

For the convenience of our patients, we use our patient portal for patient registration.  The required health information can be entered at your leisure, prior to your appointment, which allows our physicians and staff to be prepared for your visit when you arrive.

For new patients, our schedulers will provide you with access information so that you can log in to the patient portal once your appointment has been confirmed.

For existing patients, please log in to your patient portal and update any health information that may have changed since your last visit.

You will be able to review your lab and test results as well as contact our office, through the patient portal, once you have set up your account.

Medical Records Release

If you have medical records from another practice that you believe will be important for your physician to review, please complete the Authorization to Release Health Information form and have that practice send them directly to us prior to your scheduled appointment.

Insurance

Our office participates in most major insurance plans* and as a service to our patients, we file all office and procedure charges for you. We will ask to verify your insurance information at each visit in order to ensure accuracy in our filing. We can not guarantee that your insurance plan will pay any portion of your bill with us.

Please be aware that many insurance plans require a referral from your primary care physician for consultations and treatment and then a separate approval for procedures. You will need to contact your primary care physician and request a referral, if one is required, prior to scheduling your appointment with our office.

We are happy to assist you in the process of obtaining approval for both office visits and procedures. Should you arrive for an appointment at our office and your referral has not been received, you will need to reschedule the appointment to a later date to allow time for the referral to be processed by your insurance plan.

Payment of Non-covered Fees

We require co-payments, payments for non-covered services and any percentage responsibility you have under your plan, at the time of service.

For uninsured patients, we also require payment in full for an office visit at the time of service.
